Survivorship Care Plan Toolkit Cover Page-Updated 7.10.15 2
 
Triage Cancer partnered with the California Dialogue on Cancer, a program of the California Department of Public Health, to develop resources to promote the use of cancer survivorship care plans.

We created a toolkit for oncology health care professionals and cancer centers to use in developing their survivorship care plan program, as well as fact sheets on cancer survivorship care plans to give patients and caregivers a clearer understanding of the value of survivorship care plans. These fact sheets are available in English and Spanish.
